SOAP Documentation Overview

This course focuses on the purpose of documentation of THW services using SOAP (Subjective, Objective, Assessment, and Plan) note format. The key elements of documentation for the SOAP note format will be covered. Good documentation supports care coordination by giving providers accurate information about the patient’s condition, treatment and services provided, and response to previous interventions, ensuring continuity of care. *You must pass each module to progress through this course.
